Pursuant to Section 4729.16(D) of the Revised Code, the State Board of Pharmacy convened a telephone conference call for the purpose of considering a summary suspension as required by Section 3719.121 of the Revised Code with the following members participating: Robert P. Giacalone, R.Ph. (President); Lawrence J. Kost, R.Ph. (Vice-President); Diane C. Adelman, R.Ph.; Gregory Braylock, R.Ph.; Suzanne R. Eastman, R.Ph.; Elizabeth I. Gregg, R.Ph.; Nathan S. Lipsyc, R.Ph.; and Dorothy S. Teater, Public Member. Also participating were Timothy Benedict, Assistant Executive Director; David Rowland, Legal Affairs Administrator; and Sally Ann Steuk, Assistant Attorney General. |

After hearing Mr. Benedict discuss the significant facts regarding the activities of the individual, Mrs. Gregg moved that the Board summarily suspend the license to practice pharmacy belonging to Tim Ivan Miller, R.Ph. (03-3-11708) due to the fact that a continuation of his professional practice presents a danger of immediate and serious harm to others. The motion was seconded by Mr. Lipsyc and approved by a roll call vote of the Board members participating (Aye-7/Nay-0). |

After hearing Mr. Benedict discuss the significant facts regarding the activities of the individual, Mrs. Teater moved that the Board summarily suspend the license to practice pharmacy belonging to Zachary Daniel Hennekes, R.Ph. (03-2-23584) due to the fact that he has admitted to an addiction to controlled substances and due to the fact that a continuation of his professional practice presents a danger of immediate and serious harm to others. The motion was seconded by Mr. Braylock and approved by a roll call vote of the Board members participating (Aye-7/Nay-0). |

After hearing Mr. Benedict discuss the significant facts regarding the activities of the individual, Mrs. Gregg moved that the Board summarily suspend the license to practice pharmacy belonging to James Joseph Parsley, R.Ph. (03-1-24212) due to the fact that he has admitted to an addiction to controlled substances and due to the fact that a continuation of his professional practice presents a danger of immediate and serious harm to others. The motion was seconded by Mrs. Adelman and approved by a roll call vote of the Board members participating (Aye-7/Nay-0). |
